Where can I put all the labels / Iformation to send a small parcel of jiffy bag to the USA?

Hello. I have to send a small Jiffy bag (with Royal Mail Airsure Small Packet) to the U.S.. Now my question is where did all the different labels go? These are the labels you need to know where to place; The destination address, the FROM address and the customs form. is this all I need to send?
